Required Forms for Accepted Companies

To assist companies in the steps for application and reimbursement, the following is a summary of the requirements. If you have any questions, please call or email Kayla at 701-328-5383

Application Steps

  1. Employer submits online application for contract to the ND Department of Commerce (DOC) to receive funding.


  2. DOC approves or denies funding for internship.


  3. If accepted, DOC sends letter, contract, and billing agreement, Student Release of Information, Request for Reimbursement Form and W-9 form.

  4. Employer returns signed materials.

  5. Career Promotion/Internship Coordinator secures Workforce Division Director's signature and sends copy of finalized contract to employer.

  6. Employer posts position on TeamND (optional)

  7. Student applies with Employer per business instructions

  8. Employer selects and notifies intern of acceptance using minimum criteria that student has a 2.75 GPA, United States Citizen, enrolled in a ND University or College and is doing an experience that corresponds with his or her academic major.

  9. Student completes online application to DOC

  10. Career Promotion/Internship Coordinator sends student and application contact verification that information has been received.

  11. Student signs the Release of Information form and sends to the Department of Commerce.

  12. If internship is for-credit, agreement is secured between student, faculty supervisor and work supervisor

  13. If student is not earning credit, within 2 weeks of employment, the employer sends DOC the completed and signed learning agreement.

  14. Career Promotion/Intern Coordinator may visit business mid-internship to review the supervisor and intern independently.

  15. Student completes the online Exit Review.

  16. Supervisor completes the online Exit Review

  17. Reimbursement is issued.
